Objective:  Provide training and assistance to support participants in living independently and/or sustaining employment


Essential Functions Include


  1. Provide training and assistance in a variety of activities to participants in their own homes including the acquisition, retention, or improvement of skills related to activities of daily living such as personal hygiene and grooming; household chores; meal preparation; shopping; personal finances; medical; transportation; and the social and adaptive skills necessary to enable recipients to reside on their own.
  2. Provide training and assistance to support participants in sustaining paid employment at or above minimum wage.
  3. Be available to participants in case of emergency
  4. If applicable, assist participants with locating appropriate housing.
  5. Meet billable hours, as required
  6. Maintain assigned participants’ records accurately and timely as required by funding sources.
  7. Complete implementation plans, quarterly summaries, and annual reports as required by funding sources.
  8. Attend quarterly and annual support plan team meetings.
  9. Complete all required trainings.
  10. Exhibit appropriate dress, good hygiene, and professional conduct that positively reflect the agency’s mission.
  11. Complete other tasks, as assigned.
